Choosing a family doctor and CAP (Catalonia)

In Catalonia, to change your family doctor, paediatrician or nurse at the CAP you follow the same procedure as for choosing another primary care centre: you ask for the application form (or download it from the website) and take it completed to the new centre, or to your own if the change is within the same CAP. If the centre belongs to the Institut Català de la Salut (ICS), you can also do it online or in La Meva Salut. The chosen professionals must belong to the same primary care team. If you are not answered on the spot, you are informed within ten working days at most; if there is no answer after that period, the request is deemed accepted.
